Sample collection and timing

  • If you've recently been unwell, have recently completed an endurance event, or are significantly dehydrated, take your test after you've recovered for the most reliable result.

Medications and supplements

  • Please let us know in the supporting information if you take corticosteroid medication (such as prednisolone) or have a thyroid condition, as these can affect cystatin C levels independently of kidney function.

Test limitations

This test does not diagnose or rule out chronic kidney disease on its own. Like any eGFR result, eGFRcys is an estimate rather than a direct measurement of kidney function, and factors including thyroid conditions, inflammation and corticosteroid medicines can affect it.

A low, borderline or unexpected result should be reviewed by a healthcare professional and may require repeat blood testing, a urine albumin-to-creatinine ratio test or further investigation. Tell the clinician reviewing your result about your creatine use, recent intense exercise, steroid use, other medicines and existing health conditions. Don’t stop taking prescribed medicines unless advised by your clinician.

What is cystatin C?

Cystatin C is a protein made by cells throughout your body. Your kidneys remove it from your blood, so its level can be used to estimate how well they’re filtering. Unlike creatinine, cystatin C is less affected by muscle mass, making it useful when a standard creatinine result may not give a clear picture.

What does cystatin C-based eGFR mean?

eGFR stands for estimated glomerular filtration rate. It’s an estimate of how well your kidneys filter waste from your blood. Most eGFR results are calculated using creatinine, whereas cystatin C-based eGFR, or eGFRcys, is calculated using your cystatin C result.

How accurate is the cystatin C test?

Cystatin C is often more accurate than creatinine, especially if you take creatine supplements or have a high muscle mass. Its accuracy can be affected by factors like thyroid conditions, inflammation, body composition, and steroid use.

Combining cystatin C with creatinine usually gives a more precise estimate of kidney function than either marker alone.

Is cystatin C better than creatinine?

Neither marker is better in every situation. Creatinine is widely used and works well for many people, but it can be affected by muscle mass, diet, exercise and creatine supplements. Cystatin C is less affected by muscle mass, so it may provide a more useful estimate for bodybuilders, highly muscular people, and some creatine users.

What can affect my cystatin C result?

Yes, an untreated underactive thyroid can sometimes lower cystatin C, making your eGFR or kidney function appear more reassuring than it is.

Some steroid medicines, like prednisolone, can increase cystatin C and reduce your eGFR result without a true decline in kidney function.

There’s no need to stop these medications before your test but make sure you mention any thyroid conditions or medications in the additional information.

I’m a bodybuilder. Is this a better test for measuring my kidney function?

The eGFRcys calculation may be a more useful test if your creatinine-based eGFR is low or borderline and you have a high muscle mass or take creatinine. However, it is still an estimate and should not be used alone to diagnose kidney disease.

Can this test diagnose chronic kidney disease?

No. One cystatin C or eGFRcys result can’t diagnose or exclude chronic kidney disease. CKD is diagnosed based on abnormalities in kidney structure or function that persist for at least three months.  

Why might creatine affect my usual eGFR result?

Creatine is converted into creatinine in the body. Taking creatine supplements can therefore increase the amount of creatinine measured in your blood, which may make a creatinine-based eGFR appear lower even when kidney filtration has not changed. A cystatin C-based calculation provides an estimate that doesn’t depend on your creatinine level.
