Hormone blood tests on HRT: what they can (and can't) tell you
Find out when oestradiol and FSH testing is helpful, and why symptoms still lead the way
If you’ve started HRT, it’s understandable to want proof that it’s working. Checking your oestradiol levels with a blood test can seem like the obvious place to look. But the reality is a bit more complicated.
An HRT Blood Test measures your hormone concentrations, but that number is only part of the picture. Current guidance is clear that HRT should be adjusted mainly based on your symptoms, rather than a hormone blood test [1,2].
Does that mean testing is pointless? Not at all. A carefully selected test can be useful for a few reasons. It can provide a reference point, raise the possibility of an absorption issue, or support a more focused conversation with your HRT prescriber. In this article, we explore when a blood test can add value to your HRT monitoring, and when it’s less useful.

What does an HRT blood test measure?
The Medichecks HRT Blood Test measures oestradiol, follicle-stimulating hormone (FSH), and progesterone. Each result provides a different piece of information:
- Oestradiol – Oestradiol is the most potent oestrogen produced naturally by the ovaries before menopause. It’s also the form of oestrogen used in many HRT preparations. The test measures the total concentration of oestradiol in your bloodstream, but not how your tissues are responding to it.
- FSH – FSH is released by the pituitary gland and helps regulate ovarian function. Levels tend to fluctuate during perimenopause and remain raised after menopause. HRT can alter FSH levels, but the result doesn’t show how well oestrogen is being absorbed or whether treatment is working.
- Progesterone – Progesterone is produced naturally after ovulation and helps prepare and maintain the lining of the womb. Some HRT uses synthetic progestogens to help protect the womb. If your HRT contains micronised progesterone, the result can vary depending on the dose, route, and time since you last took it. A single result cannot confirm that the womb lining is adequately protected or that your hormones are in the right balance.
Can a blood test tell you whether your HRT dose is right?
Not reliably, and not on its own.
There’s no specific oestradiol target level that guarantees good symptom control. The number also can’t tell your prescriber whether increasing the dose will help. More oestrogen doesn’t always improve symptoms, and a higher dose can cause side effects or may even make symptoms feel worse.
If you’re perimenopausal, it’s further complicated by the fact your ovaries will still be making their own oestradiol. So, there’s no way of knowing how much oestradiol is coming from HRT and how much is the body’s own supply.
Oestrogen contributes to bone protection, but again, there’s no single target that everyone must aim for. If osteoporosis treatment or prevention is a specific goal, hormone results need to sit alongside clinical risk assessment and, where appropriate, bone density scanning.
This is why your prescriber will tend to focus on symptoms, side effects, bleeding pattern, treatment type, and the overall balance of benefits and risks, rather than a single hormone result.
Why can’t one oestradiol result give a clear answer?
There are several reasons.
It’s a snapshot
Oestradiol levels can fluctuate substantially, particularly during perimenopause. So, a result taken on one morning can’t show whether levels have been rising and falling over previous days.
It can’t separate your oestradiol from your HRT
Blood tests measure the total oestradiol in your blood. During perimenopause, they can’t distinguish oestradiol made by your ovaries from oestradiol absorbed from HRT. This makes the result particularly difficult to interpret unless it is unexpectedly very low.
Blood concentration is not the same as biological effect
Oestradiol works by binding to receptors in tissues throughout the body. The number and distribution of these receptors, and how the tissues respond to them, vary between people. For example, a 2025 study of 1,508 women taking HRT showed that oestradiol levels ranged from 55 to more than 2,000 pmol/L. And roughly a quarter of those on the highest licensed dose still had levels that would be considered ‘inadequate’ [3].
Route, timing, and absorption all affect the number
The result may change depending on whether you use tablets, patches, gel, spray or an implant; when you last took or applied your treatment; and individual differences in absorption and metabolism. Even people using the same product at the same dose can have markedly different results.
There’s no universal target
No serum oestradiol threshold has been shown to guarantee relief from menopausal symptoms. There’s also no agreed blood level that confirms adequate bone protection or shows that an HRT regimen is safe.
When might hormone testing be useful on HRT?
Testing has a clearer role when there’s a specific question that needs answering. A blood test may be helpful in the following scenarios.
Possible poor absorption of transdermal HRT
If symptoms continue despite careful use of a patch, gel or spray, an unexpectedly low oestradiol result may raise the possibility of poor absorption. It can prompt a review of how the treatment is applied, whether doses have been missed, the timing of the sample, or whether the delivery method is still suitable.
A reference point for comparison
A result gives you a baseline or reference point. If you repeat the test using the same laboratory, treatment, and sampling conditions, a trend can add more context than a single result.
It may help you and your prescriber see whether your level has changed while other important details, particularly your symptoms and side effects, are reviewed alongside it.
An unexpected result
Sometimes a result is far higher or lower than expected. Repeating it under controlled conditions may help distinguish a persistent finding from variation, timing, or contamination. Your clinician can then decide whether any further assessment is appropriate.
Specialist monitoring
Blood testing has a more established role with oestradiol implants. In these cases, specialists may check that levels aren’t still significantly raised before another implant is inserted. It may also be used selectively in specialist care for premature ovarian insufficiency (POI) or osteoporosis, alongside symptoms, bone health and other risk factors.
Does your type of HRT change what a blood test can tell you?
Yes. How you take your HRT affects both how much oestradiol reaches your bloodstream and how meaningful a blood test result is. The table below summarises this.

Can a blood test diagnose perimenopause or menopause?
For most people, a blood test isn’t necessary. For otherwise healthy women aged 45 or over, menopause is diagnosed based on symptoms alone.
There are two situations where a blood test may be helpful:
- Under 40s – if premature ovarian insufficiency (POI) is suspected, FSH testing is an important part of diagnosis [1,2].
- Ages 40–45 – NICE advises that FSH testing should be considered if you have menopause symptoms, including a change in your menstrual cycle [2].
Outside these groups, a normal FSH result doesn’t rule out perimenopause, and a raised one won’t tell you how far along you are or how severe your symptoms will become.
How can you get a more reliable result?
If you and your clinician decide that testing could be useful, there are some things that can affect the accuracy of your results.
How to get the most reliable result:
- Record the HRT preparation, dose, and when it was last taken or applied.
- If comparing results, use the same laboratory and similar sampling conditions where possible.
- Follow the instructions supplied with the test.
- Don’t change your treatment simply to prepare for the test unless your clinician has told you to do so.
Hormone gels, sprays, and pessaries can contaminate a finger-prick sample if the product has touched the sampling finger within the last four weeks. This can produce a falsely high result even after washing your hands. Apply these products with the opposite hand or use gloves.
Biotin can also interfere with some laboratory assays. If you take a supplement containing biotin, check with your prescriber about whether it needs to be paused beforehand.
Should you get a hormone blood test on HRT?
This is a personal and clinical decision, and it helps to know what you’re weighing up.
What a hormone blood test on HRT generally can’t do:
- Confirm your HRT dose is right – there’s no target level that guarantees symptom control.
- Diagnose where you are in the menopause transition or predict how long symptoms will last.
- Distinguish your own oestrogen from HRT oestrogen if you’re perimenopausal.
Where it tends to help:
- As a baseline reading when you start HRT or change dose, to compare against later results.
- To help flag a possible absorption issue, particularly if your symptoms suggest HRT isn’t working, despite a reasonable dose.
- To explore a previous result that was unexpectedly high or low.
- To support a conversation with your prescriber if symptoms persist despite higher-dose treatment.
- If you’re using an oestradiol implant that requires specialist monitoring.
- As part of a specialist assessment for premature ovarian insufficiency or a specific bone-health concern.
The bottom line
Symptoms and how you’re actually feeling day to day are the most reliable guide to whether your HRT is doing its job.
Before testing, it helps to ask: “What question am I hoping the result will answer, and would the result change what happens next?” If neither answer is clear, a clinical review is likely to be more useful than a blood test.
