The Enhanced Liver Fibrosis (ELF) Check is a non-invasive blood test to help identify early stage or significant liver disease which may not show up in normal liver function tests.
Liver disease has risen sharply over the past few decades and is now the third most common cause of premature death in the UK. Standard liver function tests often do not pick up the presence of liver disease (particularly in the early stages) and NICE now recommends the Enhanced Liver Fibrosis Check to screen for or monitor advanced liver fibrosis in people diagnosed with Non-Alcoholic Fatty Liver Disease (NAFLD).
The Enhanced Liver Fibrosis (ELF) Check tests for three biomarkers which are used to calculate an ELF Score. This score can determine the presence and severity of liver fibrosis even in people with normal liver function. The markers used to produce the ELF Score include hyaluronic acid, procollagen III amino terminal peptide (PIIINP) and tissue inhibitor of metalloproteinase 1 (TIMP-1). The benefits of this test are now recognised by NICE in the identification of early or significant liver disease and it provides a cost effective and non-invasive alternative to biopsy.
NICE recommend this test to screen for and monitor liver fibrosis in people diagnosed with NAFLD - one of the most prevalent forms of liver disease, particularly amongst the young and those with diabetes.
It is also used to identify the presence of fibrosis caused by hepatitis, metabolic syndrome, excessive alcohol consumption and smoking.
A healthy liver is very soft but once fibrosis occurs the liver will become firmer, eventually becoming completely hard if cirrhosis has developed.
