Cancer of the bladder results in approximately 5000 deaths per year in the UK. The incidence of bladder cancer is three to four times higher in men than women, and roughly twice as high in white men as black men. Cigarette smoking markedly increases the risk of bladder cancer - with nearly half of all new cases of bladder cancer occurring in current or former smokers. Occupational exposure to chemicals used in the dye, leather, tyre and rubber industries has also been associated with an increased risk of bladder cancer. Early detection and treatment may prevent progression to invasive disease or allow for more effective treatment of non-invasive tumours.
This urine test looks for a protein called NMP22. This is a nuclear matrix protein, found in urine. Healthy people have low levels of NMP22. People with bladder cancer, however, can exhibit noticeably high levels. The urine test uses NMP22 as a 'marker' for the detection of bladder cancer. Although it will never replace cystoscopy (an internal examination), it can help to provide a more accurate diagnosis.
Like many tumour marker tests, this test is not infallible. It is possible to have normal levels of NMP22 yet still have bladder cancer. It is also possible to have elevated levels for innocent reasons e.g. extreme exercise prior to taking the test or a recent cystoscopy. Its use as a screening test is therefore not established and it is more commonly used in the diagnosis of people who have symptoms, or to test for recurrence of bladder cancer.
This urine test is suitable for people who have been treated for bladder cancer and wish to monitor their levels of NMP22 for recurrence. It may also be useful for people who have an increased risk of bladder cancer either because or a history of smoking or because of exposure to certain chemicals at work.
